I confirm that I have voluntarily elected to receive treatment provided by Jamie at Windsor Sports Therapy. I acknowledge that the services offered may include, but are not limited to, massage therapy, dry needling (also referred to as medical acupuncture), assisted stretching and joint mobilisations.

I understand that treatment provided by Jamie at Windsor Sports Therapy is intended to support the management of musculoskeletal conditions, including but not limited to muscular tension, joint discomfort, soft tissue restriction, stress reduction and general physical well-being. I acknowledge that such treatment does not constitute medical diagnosis, medical treatment or medical advice, and is not a substitute for care provided by a qualified medical practitioner.

I acknowledge that treatment may involve physical contact, manual manipulation of soft tissues and, where appropriate, the insertion of fine, sterile needles. I understand that, notwithstanding the practitioner’s qualifications and insurance, soft tissue therapy and dry needling carry inherent risks, including but not limited to temporary soreness, bruising, discomfort, fatigue or minor bleeding.

I confirm that I am responsible for providing accurate, complete and up-to-date information regarding my medical history, injuries, medications and any other health-related conditions that may affect my suitability for treatment. I understand that failure to disclose relevant information, including known contraindications published by Windsor Sports Therapy, may increase the risk of adverse effects.

I acknowledge that Jamie at Windsor Sports Therapy does not provide medical diagnoses. Treatment decisions and recommendations are based on clinical assessment, professional judgement and the information provided by me, and do not replace advice, diagnosis or treatment from a registered healthcare professional.

To the fullest extent permitted by law, I agree that Windsor Sports Therapy and Jamie shall not be held liable for any injury, loss, damage or adverse reaction arising from or in connection with treatment, except where such liability results from proven gross negligence or wilful misconduct.

I hereby release and discharge Windsor Sports Therapy and Jamie from any and all claims, demands, actions or liabilities arising from my participation in treatment, including claims based on negligence, save where liability cannot lawfully be excluded.

I confirm that I have read and understood this disclaimer in full, including all information relating to contraindications. I voluntarily consent to receive treatment from Jamie at Windsor Sports Therapy, both now and in the future, in accordance with the terms set out above.

This disclaimer, and any dispute or claim arising out of or in connection with it (including non-contractual disputes or claims), shall be governed by and construed in accordance with the laws of England and Wales.
